A. An adoption agreement (1 copy);
B. A notice of travelling to China for adoption (1 original) ;
3. Household register and ID cards of both the adoptee and his/her guardian (1 original only for verification);
4. Material on marital status of the adoptee's guardian (1 original only for verification);
5. An agreement of compliance with family planning regulations signed between the adoptee's guardian and a local family planning department (1 copy);
6. Birth medical certificate of the adoptee (Notarized certificate on collateral offspring relationship within three generations of the adopter and the adoptee) (1 copy);
7. Declaration on consent of adoption by the adoptee's guardian (1 copy);
8. Physical examination certificate of the adoptee (1 copy);
9. An adoptee above the age of 10 may provide a declaration of willingness to be adopted (1 copy).
10. A color photo against a single-colored background (1 copy).
