(1) The original certificates of the 3 full-time booking agents at the time of the brokerage institution original establishment and a photocopy of each original certificate. The photocopies shall be affixed with official seal and the originals returned.
(2) The booking agent's employer must be a specific entity. The name of the employer shall be consistent with that in the submitted materials, and the certificate period shall be within the valid period. If the employer name on the booking agent qualification certificate is inconsistent with the name of the institution applying for establishment, and the bokking agent qualification certificate only states the region without specifying the specific employer, the agent shall apply for employer alteration to the certificate issuing authority.
(3) If there is any alteration in the original booking agents, the new booking agent certificate shall be provided. The application shall only be accepted when there are 3 booking agent certificates.
4. An original authorization letter (If a non-legal representative comes to handle the matter of application, the photocopy of the handling agent's ID card shall be submitted and pasted.）
